The mission of Casa de Salud is to facilitate and deliver high quality medical and mental health services for uninsured and underinsured patients, focusing on new immigrants and refugees who face barriers to accessing other sources of care.
Programs or services
Founded in St. Louis in 2010, Casa provides culturally and linguistically responsive primary care, behavioral health services, and patient navigation. Through our clinic, GUIA (GUIDE) case management program, and Mental Health Collaborative (MHC), we reduce cost and language barriers, strengthen continuity of care, and help clients access services within the broader health system.

Primary Health Care Clinic: Our culturally competent staff partners with volunteer medical providers to provide care to underserved immigrants. The Clinic is the entry
point for most folks who come to Casa. Patients receive primary and acute care from volunteer doctors, nurses, nurse practitioners, and physical therapists, among others. The Clinic also provides chronic disease management and health literacy education, lab testing, cancer screening, prescription assistance, women's health care, and other services.

Case Management: The GUIDE program addresses the need for comprehensive health care follow-up in the immigrant community. GUIDEs are high-touch and expert case managers who accompany patients and clients along their healthcare journey, ensuring they understand the patient-provider relationship and gain confidence in seeking out the care they deserve. GUIDEs also nurture partnerships with external partners to extend access to care for patients whose health needs exceed Casa's ability to provide care.

Mental Health Collaborative (MHC): The MHC is a partnership of Casa staff, mental health agencies, and therapists working together to develop and deliver inclusive behavioral health services to high-need populations. All MHC clients are triaged by our bilingual and culturally competent staff and volunteers. Through this innovative
approach, expert therapists across a wide range of modalities will connect to those in need while developing strong cultural competency skills. Casa staff also facilitates learning sessions and case conferences to promote knowledge and practice sha
Target Audience
Immigrants and refugees face significant barriers to accessing healthcare, resulting in delayed care, unmanaged chronic disease, and serious mental health needs. Many are uninsured or underinsured and work in low-wage jobs without benefits, making preventive and ongoing care financially inaccessible. Cultural unfamiliarity with Western medicine, fear of unknown medical costs, and language barriers further discourage individuals from seeking care until conditions become urgent.

Mental health needs within immigrant communities are especially acute. A Casa assessment of 80 foreign-born patients found that 86% met criteria for PTSD, 86% experienced depression, and 91% experienced anxiety or panic attacks. Nearly half (49%) reported experiencing more than five traumatic life events. These challenges are often compounded by the stress of migration, economic insecurity, and social isolation, yet access to culturally and linguistically responsive behavioral health services remains limited.

Casa primarily serves uninsured immigrants from over 45 countries and 20 counties across Missouri and Illinois. The average patient age is 39; 55% identify as women and 45% as men, and the majority identify as Hispanic/Latino. Over half of patients work in hospitality, with another 15% employed in industrial, agricultural, construction, and other physically demanding jobs that offer low wages, unstable hours, and little access to health insurance.

Casa addresses these needs through an integrated model of primary care, mental health services, and patient navigation. Through its clinic, GUIA case management program, and Mental Health Collaborative, Casa provides culturally responsive medical care, trauma-informed therapy, and support navigating referrals, specialty care, and financial assistance.
